Rationale for Simultaneous Operations on The Abdominal Organs and The Abdominal Wall in Patients with A Ventral Hernia

Abstract: Introduction. According to the World Health Organization, "Hernias of the anterior abdominal wall in combination with simultaneous diseases of the abdominal organs are one of the most common surgical diseases and occurs in 3-7% of the population, with a predominant lesion of people of working age". The aim of the study was to substantiate the tactics of surgical treatment of patients with ventral hernia and concomitant pathology of the abdominal organs, aimed at reducing the frequency and severity of complications after simultaneous operations. Material and research methods. The study was based on 197 patients with ventral hernia admitted to the surgical department of the 1st clinic of the Samarkand State Medical Institute in the period from 2012 to 2020. Patients were conditionally divided into two groups: 47.2% of patients with hernia of the anterior abdominal wall made up the comparison group. The main group consisted of 52.8% of patients. In all patients of the main group, the main surgical disease was ventral hernia. In addition, the main group of patients had concomitant surgical pathology of the abdominal wall and abdominal organs. Results. Long-term results were analyzed in 151 (76.6%) patients out of 197 operated patients for ventral hernia and concomitant simultaneous pathology of the abdominal organs. Out of 151 patients examined in long-term periods, recurrence of ventral hernia was noted in 5 (3.3%), while in the comparison group this indicator was 4.3% (3 patients), and in the main group - 2.4% (2 patient). These results were obtained due to the use of the above measures for the prevention of relapse of the disease with the transition to a differentiated choice of surgical intervention individually for each patient, as well as the addition of the operation, if necessary, by performing dermatolipidectomy in obese patients. Conclusions. The implementation of the simultaneous stage of the operation as a whole did not negatively affect the results of surgical treatment of patients with ventral hernia and simultaneous pathology of the abdominal organs. At the same time, the deliverance of the patient from several diseases within the framework of one anesthetic aid and surgical intervention justifies the need to perform simultaneous operations.
