Complications After Regenerative Periodontal Flap Surgery - A Retrospective Cohort Study

Regenerative periodontal therapy is one of the promising methods to restore lost periodontal structure and functional attachment through reconstruction of cementum, periodontal ligament and alveolar bone. However the use of different regenerative tools may also end up in complications like the membrane exposure, graft exposure, tissue/flap necrosis, post operative pain/ swelling etc that hinder the regeneration. The aim of this study is to determine the prevalence of complications after the regenerative periodontal flap surgery and to assess the patient and clinical characteristics associated with complication. A retrospective evaluation was conducted to find out subjects who had undergone the regenerative periodontal flap surgery between June 2019- March 2020 at the University Dental Hospital. For the included subject, data regarding complications if present, demographic factors, other clinical factors associated with the complication were extracted. Out of the 242 subjects who underwent regenerative therapy, the complications were rarely present with only 6 developing it. The most commonly reported complication was the post operative pain which was seen in 2.1% (n=5) followed by membrane exposure 0.38% (n=1). Out of all the comorbidities assessed males, mandibular posterior region and bone grafting were found to be associated with increased prevalence of complication. From the results it's understood that the complications after regenerative periodontal therapy are generally rare, with only post operative pain/ swelling being 2.07% prevalence. However considering the retrospective nature of the study, future prospective longitudinal RCTs are necessary to identify the actual incidence and factors associated with it.
