Reliability And Comparison Of Yen-Angle, W-Angle And B Angle In Assessing The Sagittal Skeletal Discrepancy Based On The Growth Patterns In Dravidian Population- A Retrospective Cephalometric Study

Abstract: Introduction: Assessing an accurate basal bone jaw base relationship is essential for proper diagnosis and treatment planning. Various methods are available to assess the antero-posterior jaw discrepancy and the most commonly used parameters include ANB angle and Wits appraisal. Beta angle is one of the more reliable indicators of jaw base discrepancies. Hence in this study, we have tried to correlate yen angle and W-angle with respect to beta angle in assessing sagittal discrepancies. Beta angle can be susceptible to variations in cases where the condylar axis undergoes rotational growth. Hence, we have also tried to assess the reliability of yen angle and W-angle in determining skeletal malocclusions in different growth patterns as well Aim: The aim of this study is to determine the mean and standard deviation values for Yen angle and W-angle with respect to Beta angle for a sample of Class I, II, III Dravidian malocclusion patients and to determine their reliability in assessing malocclusions irrespective of the growth patterns. Materials And Methods: 60 pretreatment lateral cephalograms were obtained as a part of this retrospective study conducted in Saveetha Dental College, Chennai. The cephalograms were obtained from DIAS software. These samples were divided into 3 groups as : Class I, II, III. Beta angle was taken as the reference for classifying the samples into the three groups. Statistical analysis was carried out by SPSS Software 20.0 (SPSS Inc, Chicago). ANOVA test of variance was carried out to check the level of significance between groups. Results: For our selected population, the mean beta angle for Class I pattern is 29.87+2.240, Class II pattern is 23.42+2.270 and for Class III skeletal pattern is 37.58+2.390. The mean yen-angle for Class I pattern is 127.79+1.990, Class II pattern is 121.86+3.950 and Class III pattern is 136.19+2.980. The mean W-angle for the Class I pattern is 58.01+2.040, Class II pattern is 54.36+2.810 and Class III pattern is 64.57+1.940. Conclusion: There is no significant difference between Yen angle and W-angle when compared to Beta angle for detecting different skeletal malocclusions. There is a higher positive correlation of Yen angle and W-angle with Beta angle in Class I and Class II than in Class III. There is a weak positive correlation between beta angle and yen angle, w-angle in vertical growers compared to horizontal and average growers. This indicates that Yen angle and W-angle are more predictable in determining skeletal discrepancies in cases of vertical growers than beta angle which is susceptible to changes seen in the condylar axis
