Periodontitis causes bone loss leading intrabony defects and periodontal regeneration is the restoration of the periodontium to its original form and function. Various treatment modalities have been used in the past for periodontal regeneration and recently it has been found that melatonin, which is an endogenous hormone, has the potential to inhibit bone resorption and augment bone mass. It also down-regulates RANK-mediated osteoclast formation and activation, thereby increasing bone mass. Besides, it is a significant free radical scavenger and antioxidant. These distinctive properties of melatonin can be used for periodontal regeneration. Also, it has been established that PRF, which is a second-generation platelet concentrate, can be used for periodontal regeneration as it contains numerous growth factors like platelet-derived growth factor and vascular endothelial growth factor. In the past, PRF has also been used in combination with bone grafts, but to our knowledge, it has not been used in combination with melatonin for intrabony defects. Therefore, in this case report, melatonin was used along with PRF in the treatment intrabony defect and positive clinical impact was seen.
