Pathophysiology of PCOS and an Update on Herbal Remedial Measures: A Review

Abstract:

Polycystic ovarian syndrome is a pathophysiological condition affecting women of reproductive age. It involves multiple clinical manifestation. According to ayurvedic perspective PCOS can be designated as ‘Arthava Kshaya’. A specific drug or complete cure for PCOS is not validated till now. Hormonal imbalances manifested includes hyper androgenism and elevated LH/FSH ratio. Excess ovarian androgen level prevent follicle growth, maturation and release of ovum. Under hormonal imbalance, the two cell-two gonadotrophin theory fails to maintain hormonal homeostatsis. Insulin sensitivity is an important factor which can stimulate the production of more LH receptors and therefore signaling the master gland for producing more leutinizing hormone. On contrary, FSH levels becomes inadequate to convert all of the produced testosterone to estrone and estradiol due to low levels of aromatase . This imbalaced state leads to a condition called hyperandrogenemia , an hallmark of PCOS. All these strategies appear to be moderately effective and usually associated with side effects. This scenario demands an alternative therapy for PCOS with less harmful effects and more curative efficiency. Indian ancient heritage contains of many herhs to reduce the complications of PCOS. In this review hormonal and cellular mechanisms involved in PCOS and herbal remedies for PCOS were discussed.
