Fibroadenoma [FA] of the breast is a benign neoplasm and is one of the most common benign breast lesions that is encountered in a surgeons clinic. FAs are commonly found in the younger age spectrum, but are also reported in elderly women. The increased rate of misdiagnosis of FA in the elderly women, its association with infiltrating breast cancer and previously reported cases of breast cancer seen to be originating from previous FA is alarming and needs awareness regarding FAs in older individuals. Gene alterations might be associated with recurrent lesions and could have therapeutic implications in the future. Here, we present a case of recurrent FA in a 70-year-old patient with an emphasis on the diagnosis along with a brief review of literature on the genetic alterations seen in FAs. Out of many gene alterations investigated in FAs, MED12 seems to be more relevant and frequently associated with FAs. Hence emphasis is made on the importance of future studies determining the role of MED12 mutations in the pathogenesis of breast FAs.
