An Approach of resistance pattern of the four common antifungal drugs Griseofulvin, Terbinafine, Itraconazole and Fluconazole in patients having Dermatophytosis

Abstract: The incidence of fungal infection is on the increase since 1950. The current focus of interest in research is the antifungal susceptibility testing. The lack of a standardized method has led to inter-laboratory variation in the absolute MIC of antifungal drugs. The clinical relevance of antifungal susceptibility testing in dermatophytosis increases with the existence of a variety of effective antifungal drugs. The last fifteen years have witnessed marked increase in the frequency of severe antifungal infections as well as in the number of available antifungal agents. The filamentous fungi causes less invasive infections than those caused by the yeasts. However, the emergence of new and less susceptible mould pathogens warrants the evaluation of their in vitro antifungal susceptibilities to both established and investigational antifungal agents. Mc Nemar’s Chi-square test to compare the proportion of symptoms before and after treatment demonstrated a p value <0.05 for all the four antifungal drugs ,signifying that all had a therapeutic benefit in reducing the signs and symptoms. However Terbinafine group had greater proportion of patients with absence of symptoms and signs.There was 100% reduction in erythema with the all four antifungal drugs used in the study. At the end of follow up , greater proportion of patients in Terbinafine group had resolution of desquamation (84%) as compared to itraconazole (52%), Griseofulvin (32%) and Fluconazole (40%) while 16% in group A,48% in group B, 64% in group C and 60% in group D continued to have desquamation at the end of treatment
