The Efficacy of Oral Ketoconazole in Reduction of Lipase Enzyme Activity of Cutibacterium Acnes in Acne Vulgaris Iraqi Patients

Acne vulgaris is the most common dermatological disease in the world. A relation between this disease and Cutibacterium acnes (Propionibacterium acnes) has long been suggested. This disease is multifactorial and the activity of lipase enzyme of Cutibacterium acnes leads to sebum destruction and resulting to evoke skin inflammation. The current study is conducted to assess the oral ketoconazole therapy for acne vulgaris patients and understanding of C. acnes’ pathogenicity; via the lipase enzyme. One hundred twenty patients with acne vulgaris were examined in the outpatient clinic of the dermatology center during the period 1/9/2019 to 1/3/2020. Bacterial samples from acne lesions were collected before oral ketoconazole therapy, only ninety- six samples were collected after oral ketoconazole therapy. The specimens were transported to the laboratory by using Thioglycolate broth. After laboratory diagnosis the bacterial samples were cultured on spirit blue media, to identify the lipase activity. All of C. acnes isolates were demonstrating strong lipase production before treatment. The degree of lipase enzyme significantly decreased after treatment with oral ketoconazole in 67 (69.8%) of isolates but 29 (30.2%) of the cases were not. The results indicate the activity of the lipase enzyme as a virulent factor produced by C. acnes samples that isolated from skin acne lesions. So, using oral ketoconazole may prevent the optimal environmental conditions for the production of lipase by this bacterium, and subsequently, oral ketoconazole therapy may help for good improvement response.
