Evaluate the efficacy of autologous serum therapy (AST) in patients with chronic idiopathic urticaria

Chronic urticaria is a common and distressing dermatosis and patients were subjected to high pill burden.
Autohemotherapy was regarded as potentially curative therapeutic option in chronic urticaria especially in
autoreactive group. Later this procedure was refined by using autologous serum instead of whole blood. The aim
of this study was to find out proportion of ASST positive patients among chronic urticaria patients and efficacy of
Autologous Serum Therapy (AST) in both ASST positive and negative groups. Chronic urticaria (CU) is a vexing
problem and patients of CU suffer from the morbidity that arise from irritable itch and weals and are also
subjected to a huge antihistamine pill burden. ASST was done to all patients who were selected for the study.
Thirty patients were given autologous serum and remaining thirty were given injection normal saline (control
group) weekly for nine weeks and advised for atleast two followup visits at 20
week. Assessed using
urticaria total severity score (TSS), among 60 pts 25% were ASST positive. TSS showed significant reduction in
AST group than the control group and the response between ASST positive and negative patients was not
statistically significant. Autologous serum therapy is effective in the treatment of CIU. ASST positive as well as
negative patients responded to AST but when compared ASST positive patients response was earlier and better
than negative patient. AST should be considered seriously for all treatment refractory cases of CIU.
