A Study Of Efficacy Of Autologous Platelet Rich Plasma In The Treatment Of Chronic Diabetic Foot Ulcers

Abstract:

Diabetic foot ulcers are a communal health problematic around the world. Often it becomes chronic and nonhealing
causing
significant
distress
to the patient physically, mentally and financially. Its management is challenging
especially in developing countries like India, where there is inadequate health resources and poor health awareness
among the public. Its global burden is increasing and various treatment modalities are available for treating diabetic
foot ulcers. One novel therapy recently gaining interest is the Platelet Rich Plasma (PRP) therapy. This study
evaluates the efficacy of PRP in the treatment of chronic diabetic foot ulcers.
A total of 42 patients were registered in the study. They were arbitrarilyseparated into two groups. One group is
the PRP Group, where glycemic control, cleansing with normal saline, debridement of the wound, application of
platelet rich plasma and treatment of infection was carried out. The other group was the Control Group, where
glycemic control, cleansing with normal saline, debridement of the wound and treatment of infection was carried
out. The ulcer was measured at the end of each week and area was calculated for both the groups. This was done
for a total duration of five weeks. The percentage improvement in area was calculated and compared for both
groups.
Male patients (71.4%) were encountered more than female patients (28.6%) in both the groups. The most common
age group affected was 51 to 60 years in both the groups (50%). The duration of ulcer for majority of the patients
was between 8 weeks and 16 weeks in PRP as well as control group (54.8%). The most common site affected was
sub-metatarsal region seen in both groups (35.7%). Majority of the patients were found to be farmers in both the
groups (28.6%). At the end of five weeks, the mean percentage development in area was found to be 82.19% in the
PRP group and 37.50% in the control group.Ulcer healing was found to be better and faster in the PRP group than
that of control group, proving the superior efficacy of platelet rich plasma therapy. Thus, Platelet rich plasma
therapy is a simple, cheap and effective day care procedure which can be used safely for treating chronic nonhealing
diabetic
foot
ulcers.
