A detailed knowledge and comprehension of the anatomy and morphology of the root canal is necessary for a healthy endodontic procedure. In variable root canal layout, most of the maxillary second premolars had one single root of about identical frequency. Equivalent incidence of double split up and fused roots with total value of one channel and two root canals correspondingly was noted. A very unusual occurrence of maxillary second premolar with three origins, two vestibular and one palatal, including one with a single canal structure, was observed. In maxillary second premolars, accessory lateral canals were also found. Transverse communications with one root and double fused roots were detected in maxillary second premolars. The most frequently observed structures in the maxillary 2nd premolar are type II and type IV vertucci. As a result, their root canal morphology did not show any significant difference between male and female. When considering surgery of maxillary second premolars, it is reasonable to expect challenges due to different channel configuration and transversal channel communications, which can not be identified with clinical and radiology tests. Hence, this study was undertaken to investigate the variation in number of roots and root canals in maxillary second premolar using Vertucci classification by comparing the morphology of the maxillary second premolar that had been reviewed extensively in various research articles.
