Chronic idiopathic urticaria (CIU) is a very communal distressing allergicskin complaint. CIU not responsive to
antihistamine and other oral drugs is difficult to treat and is a therapeutic challenge to a dermatologist. This work
focuses to appraise the effectiveness of autologous serum therapy (AST) in patients with chronic idiopathic
urticaria and to compare Autologous serum skin test positive and negative patients response to autologous serum
therapy. Sixty patients with chronic idiopathic urticaria not responding to other modalityof treatment were
selected for the study. All care was taken to follow strictly the inclusion and exclusion criteria. ASST was done to
all patients who were selected for the study. Thirty patients were given autologous serum and remaining thirty
were given injection normal saline (control group) weekly for nine weeks and advised for atleast two followup
visits at 20
week. Assessed using urticaria total severity score (TSS), among 60 pts 25% were
ASSTpositive. TSS presentedimportant reduction in AST group than the control group and the response between
ASST positive and negative patients was not statistically significant. Autologous serum therapy is operative in the
treatment of CIU. ASSTpositive as well as negative patients responded to AST but when compared ASST positive
patients response was earlier and better than negative patient. AST should be considered seriously for all
treatment refractory cases of CIU.
