Among Tridosha Vata is only responsible for the all Cheshta or movements in the body. Mainly Vyana Vayu controls all body movements. Due to any type of causes, if the functions of Vata Dosha gets disturbed, it will further disturb the functions of body and may cause different types of diseases in the body. One of the important disease of vitiated Vata Dosha is Pakshaghata. Due to similarity in signs and symptoms, Pakshaghat is correlated with Hemiplegia according to modern science. According to classics, Basti is main line of treatment for all types of Vata Roga or diseases and in the specific line of treatment of Pakshaghata it is mentioned that “Pakshaghate tu Virechan” i.e. they mentioned the Sneha or Snigdha Virechana in Pakshaghata. Before the administration of any Basti, one has to do Koshta Shodhana for getting maximum benefits from Basti Karma. By keeping a view on all these theories, we are going to present a case study where all these above mentioned theories are incorporated in a patient who appeared to Parul Ayurved Hospital with history of loss of movements in left half of the body, weakness in left half of the body, difficulty in walking since 10 days. As patient got ischemic stroke attack 10 days back and got paralyzed on left half of the body, initially patient took treatment in allopathy Hospital for 10 days. After the discharge the patient came to Panchakarma OPD of Parul Ayurved Hospital, for the further management. In this case study, patient was treated with Koshtha Shodhana in the form of Mridu Virechana followed by Matra Basti along with Shamana Aushadis for one month. This simple treatment protocol shown good improvements in signs and symtoms of the patient and also helps us to understand the importance of Koshtha Shodhana prior to Basti Chikitsa.
