Management of open tibial fracture in children : methods of treatment, assessment and evaluation of the outcome in our society

Abstract: Background: Management of compound tibia fractures remains a challenge for the orthopedic surgeon, not only in adult but also in children. In adult patients, the problems have been studied widely, and numerous published studies on these injuries have focused on the preferred treatment and outcome, and so detailed treatment protocols have been developed, while in children there was no clear guidelines regarding the management of open tibial fractures exist. Aim of this study is evaluate and assess our policy and the protocols for dealing with such type of injuries in children, analyzing the results, the complications and determine the outcome. Method and results: A cross sectional prospective analytical study including forty six child with the open tibial fractures, were managed and treated between September 2013 and October 2020 in Al- Husain teaching hospital of Nasiriya city-Iraq. The average duration of follow up was (10) months, range (3-36) months. Gustilo–Anderson classification is used to recorded the different grading and severity of fractures to Type I 15 patients (32.60%) ; Type II 20 patients (43.47%) ; Type IIIA 7 patients (15.21%) ; Type IIIB 4 patients (8.69%). Conclusion: Age, gender, mechanism of injury were not independent factors for the determination of the complication, delayed union and non- union mainly affected by Gustilo et al. class of fracture grading, where the percentages increase with the grading advancement. The outcomes not differ from that of the adult tibial fracture. Type of method of fracture fixation not demonstrating the outcome
