Occurrence of Plasmid-Mediated carbapenem Resistance genes among Pseudomonas aeruginosa isolated from clinical and hospital environmental samples in Al –Nasseryia city, Iraq.

Abstract: Background: Multi-drug resistance Pseudomonas aeruginosa is one of major causes of nosocomial infections and is associated with infections of wounds, burns, urinary tract infections, and pneumonia. One of the most common pathogenic bacteria causes a wide range of infections in burn patients is Pseudomonas aeruginosa. There are different mechanisms of resistance are expressed simultaneously against the antibiotics such as production of antibiotic-inactivating enzymes (carbapenemase enzyme). Aim of the study: In this study, the mechanism caused by the occurrence of carbapenem resistance genes that are associated with production of inactivating enzymes is going to be investigated Materials and methods: A total of 404 isolates collected from clinical and hospital environment samples during the period of study from February 2019 to November 2019. The VITEK-2 compact system was used as a confirmatory test for final identification of eighteen P. aeruginosa isolates and also was used for antibiotic susceptibility test by using AST-N222 cards. The molecular detection of carbapenem-resistance gene performed by using of PCR technique with specific primes for class A carbapenemase (Serine B-lactamase): blaKPC, and blaGES genes, class B (Metallo-B-Lactamase): blaIMP, blaVIM, blaSPM, blaGIM, blaSIM, blaNDM genes, and class D (Carbapenemase):blaOXA 50, blaOXA-40, and blaOXA-198 genes for all the 27 carbapenem-resistance P.aeruginosa that isolated from clinical and hospital environmental samples. Results: The results of PCR showed that 20 (74.1%) isolates with positive PCR products for at least one resistance genes. Some of these isolates harboring a combination of more than one gene. The blaOXA-50gene was the most common gene which appear in 19 (70.3%) isolates, 2(7.4%) of them was appeared in combination with bla OXA-40. The blaVIM gene was detected in 4 (14.8%) isolates, followed by 2(7.4%) isolates with blaSPM gene. While blaGES gene appear in 1(3.7%) isolate only. While no PCR product was showed for the blaIMP blaGIM, blaSIM, blaKPC, blaNDM, andblaOXA-198 genes on the electrophoresis gel. The results showed a strong relationship between the isolates that carrying blaOXA-50gene and their ability of XDR characteristics. Conclusion: The most finding of carbapenem-resistance genes detected by molecular methods (PCR technique) are blaOXA-50 and blaVIM and the cause of concern is the presence, for the first time, the genes like blaGES and blaSPM in Al-Nasseryia city, While, the blaIMP blaGIM, blaSIM, blaKPC, blaNDM, and blaOXA-198 genes were not detected in our study.
