Anterior teeth wear leads to the loss of anterior guidance. Anterior guidance functions to protect the posterior teeth from wear or any other damage during various excursive movements. Once the posterior teeth collapse there is an evident loss of normal occlusal plane and eventually there is reduction in the occlusal vertical dimension. This case report describes a 54 year old male, who had anterior guidance anterior guidance loss, severely worn of dentition, and resultant reduction of the vertical dimension. Anatomical landmarks, facial and physiologic measurements were used to decide the amount of vertical dimension raise is required. Oncem the new vertical dimension was been confirmed, interim restoration followed by the permanent reconstruction was initiated. The unilateral distal extension was treated by providing a removable attachment prosthesis This case reports that a satisfactory clinical result was achieved both esthetically and functionally after raising the vertical dimension.
