Author: JAFFAR HUSSAIN, VIRENDRA KUMAR, DR RAVISHANKAR, DR RAMYA
Abstract: Drug related problems were found to have higher incidence among chronic kidney patients due to polypharmacy. A Drug-Related Problem is an occasion or condition connecting drug therapy that really or possibly delays by anticipated health consequences. The main objective of our present study was to identify number and type of drug related problems and to demonstrate the importance of clinical pharmacist intervention in ensuring rational drug therapy and improving therapeutic outcome in chronic kidney disease patients. A Prospective observational study was carried out in chronic kidney disease patients of 350 bedded private multispecialty hospitals. The type, Number and incidence of DRP’s were identified and documented according to PCNE classification v.6.2. A total of 152 patient case sheets were evaluated during study period of 6 months, out of which 224 DRPs were identified in 108 patients. The most common drug related problem was therapeutic failure due to drug interactions, overdose (11.53%), and mild incidence of adverse drug reactions. The current study demonstrates that majority of drug related problems are preventable. So continuous identification, monitoring and intervening improves therapeutic outcome and patient quality of life.
