Diffuse large B-cell lymphoma (DLBCL) is an aggressive type of non-Hodgkin lymphoma. It can involve different organs and the endocrine system is less reported to be a primary site for DLBCL. This paper reports a case of bilateral adrenal gland involvement with DLBCL. An 85-year-old man from Yasuj, Iran who presented to a referral training hospital with the chief complaint of abdominal pain, since 10-12 days before admission. This pain was initiated from the epigastrium and had no dissemination to other areas. After the initial laboratory and imaging assessments, the patient underwent bilateral adrenalectomy for tumoral mass excision. The pathologic study of mass showed non-germinal center type of DLBCL in both sides. Regarding the presented case, adrenal gland may bilaterally be the first location of DLBCL
