Intra and post-operative complications of semi-rigid ureterorenoscope with pneumatic lithotripsy.

Abstract: Ureterorenoscopy (URS) is a popular and growing option for management of ureteric and renal stones, now Ureteroscopy has gradually become a major technique for the diagnosis and treatment of lesions of both the ureter and the intra-renal collecting systems Newar 2018. Intra-operative complications, including (uneventful, bleeding, ureteral perforation, failed access, conversions, and avulsion). Post-operative complications are bleeding, fever, urinary tract infection (UTI), pulmonary embolism, cerebro-vascular accident (CVA), sepsis, acute abdomen, or acute myocardial infarction (AMI) B. K. Somani,2016. The overall complication rates range from 3.5% 26.3% to 57.9% Newar 2018. There are many classification system for evaluation URS complications, in which Clavien–Dindo classification system is the most widely used scoring of complications in surgical procedures. However, both the Satava and PULS scoring systems are URS specific B. K. Somani,2016. in our study we try to categorized the incidence of URS complication in 85 patients (23 females, 62 male) presented with obstructed ureteric stones and intervened with URS lithotripsy procedure. The complications classified into:Minimal trauma ( injury to ureteric orifice, minimal perforation by guide-wire, lacerations of epithelium).,Major trauma ( perforation by URS tip ,stone extra-ureteric migration, avulsion of ureter),Bleeding either minor of major .( urethral and prostate source , ureteric source).Fever, Cardiopulmonary complication ,Conversion to open procedure.Results show that 30 (35.2%) patients had minimal trauma, and 1 patients has major trauma(1.1%), 40 patients has bleeding (47%) , 23 patients has fever(27%), 1 patients had cardiopulmonary complications(1.1%), 2 patients converted to open surgery(2.2%). We concluded that URS is safe procedure for stones management in which most of the complications are minor and tolerable for the patients with overall complication major complication rate of 1.1%
