THE INDICATORS OF SIGNAL-AVERAGED ECG VENTRICULAR COMPLEXES OF PATIENTS WITH ARTERIAL HYPERTENSION AND SLEEP APNEA SYNDROME

Abstract: To evaluate the impact of sleep apnea syndrome on the parameters of signal-averaged ECG ventricular complex, the structure and the number of arrhythmias of patients with moderate hypertension.90 patients were examined with 2nd and 3rd degree of severity hypertension - 58 males (average age of 48.2 ± 5.4 years) and 32 females (average age of 53.6 ± 4.4 years). All patients underwent 24-hour ECG monitoring in 12 leads with pneumogram recording (thoracic and abdominal), evaluation of the signal-averaged ECG of the ventricular complex. Depending on the severity of sleep apnea, patients were divided into 3 groups: 1st group (absent OSA) - 26 people, 2nd group (mild OSA) - 42 people, 3rd group (middle degree OSA) - 22 people. Groups of patients with OSAS were characterized by higher indicators of QRS and LAS40, smaller indicators of RMS40, which reflected the increased electrical instability of ventricular myocardium. LVP detection frequency from patients who does not suffer from OSA, was minimal - 8%. With increasing severity of OSA increased the number of patients with VLP - from 21% in the group with mild degree OSA to 73% in the group with an average degree of OSA. The frequency of detection of VLP patients with OSA was higher in nighttime. The frequency of detection of ventricular arrhythmias was also connected with the degree of severity of OSA – the largest number of patients with ventricular extrasystoles and paroxysms of ventricular tachycardia was noted in the group of patients with an average severity of OSA. The average number of ventricular arrhythmias detected during the day was also connected with the severity of OSA.To identify the groups with a high risk of arrhythmic developments and further observation of patients with arterial hypertension, it is reasonable to conduct the daily monitoring of ECG with the possibility of SA-ECG test and evaluation reopnevmogrammy.
