Therapeutic Isoniazid Monitoring in Tubercular Patient Using High Performance Liquid Chromatographic Technique

In this study the concentration of Isoniazid in the plasma of Tuberculosis patients who were taking Isoniazid alone or along with other medication was determined using high performance liquid chromatography. To develop a precise, accurate and reproducible high performance liquid chromatography (Shimadzu 20A Prominence HPLC System) method for the estimation of Isoniazid in plasma, various sample preparation methods were tried and the proposed chromatographic condition was found appropriate for the quantitative determination. Chromatographic column was used 250x4.6mm phenomenex C18 with a particle size of 5mm. The mobile phase comprised of methanol: buffer (60:40). Sample of 20patients was analysed and all of them were found to be in the therapeutic range. Out of 20 patients 18 are slow acetylators and 2 were fast acetylator i.e. 90% was slow acetylators and 10% were fast acetylators. Isoniazid concentration in patient’s plasma for slow acetylators was found to be 3-5 mg/ml & Concentration Range in Plasma for fast acetylators was 1-2 mg/ml. The p- value is < 0.001, considered extremely significant. Confidence interval is 95% in the interval of difference 3.130 to 4.073 p value is >0.10. The data passed the normality test with >0.05. Mean = 3.602, No. of samples = 20, Standard deviation =1.008 Standard deviation error = 0.2253, Minimum concentration =1.380, Maximum concentration = 4.870, Median = 3.695, Lower 95% confidence interval = 3.130, Upper 95% confidence interval = 4.073. The standard curve was linear and coefficient of co-relation was found to be greater than 0.99 throughout the study.
