Phyllanthusacidus(L.) Skeels, commonly known as star gooseberry or Otaheite gooseberry, is a medicinally important tropical plant belonging to the family Phyllanthaceae. It has been extensively utilized in traditional systems of medicine for the management of various ailments, including fever, inflammation, diabetes, liver disorders, skin diseases, gastrointestinal disorders, and infections. This review provides a comprehensive overview of the botanical characteristics, systematic classification, vernacular names, phytochemical composition, and pharmacological properties of P. acidus. The plant is rich in bioactive phytoconstituents such as flavonoids, phenolic compounds, tannins, lignans, terpenoids, alkaloids, saponins, glycosides, and vitamin C, which contribute to its diverse therapeutic potential. Experimental studies have demonstrated significant antioxidant, antimicrobial, anti-inflammatory, analgesic, antidiabetic, hepatoprotective, antihyperlipidemic, gastroprotective, wound-healing, neuroprotective, antipyretic, and anticancer activities. These pharmacological effects support many of its traditional medicinal applications and highlight its promise as a valuable source of natural therapeutic agents. However, despite encouraging preclinical findings, clinical evidence remains limited, and further investigations are required to identify active constituents, elucidate molecular mechanisms, establish safety profiles, and validate clinical efficacy. Overall, Phyllanthusacidus represents a promising medicinal plant with considerable potential for the development of evidence-based herbal medicines, functional foods, and novel pharmaceutical agents.
