Optimizing Diagnosis and Management of Acute Appendicitis in Adults: The Role of Appendix Diameter on Ultrasound and RIPASA score

Abstract:

Background:Acute appendicitis is a common reason for emergency room visits, and prompt, accurate diagnosis and treatment plan is crucial for improving patient outcomes. Early recognition and management, including surgical intervention, are key to preventing complications like rupture and peritonitis.
Methods:We conducted a prospective study following ethical approval, enrolling adult patients who were clinically suspected of having acute appendicitis during a time period of 6 months at a tertiary care institute. Each patient underwent assessment using the RIPASA (Raja Isteri Pengiran Anak Saleha Appendicitis) score and imaging studies. Clinical, biochemical and imaging findings were then correlated with surgical outcome to determine diagnostic accuracy of these parameters.
Results:The combination of the RIPASA score, elevated leukocyte counts, and increased appendix diameter on ultrasound significantly improved the diagnostic accuracy for acute appendicitis. Correlation with surgical outcomes confirmed the utility of these parameters in accurate diagnosis of Acute appendicitis in adults.
Conclusions: The integration of the RIPASA score, blood parameters, and ultrasound findings enhances the diagnostic accuracy for acute appendicitis in adults, providing a reliable and efficient diagnostic protocol in clinical practice.
