Evaluation Of Antihypertensive Utilization And Assessment Of Framingham Risk Score To Predict Cardiovascular Risk.

Abstract: AIM AND OBJECTIVE: This study aims to identify the prescribing pattern and utilization of antihypertensive drugs and determine its adherence with JNC-8 guidelines. The primary objective of the of the study is to understand the demographics of hypertensive population for rational use of drugs and better healthcare outcomes. The study estimates individual 10-year of developing cardiovascular disease using Framingham risk assessment so as to provide an insight of the patient’s condition. METHODOLOGY: A prospective, observational study was carried out in the out-patient department of a 300 bedded tertiary care, Shadan medical and teaching hospital situated in Hyderabad for over a 6 months period. A total of 100 patients who were previously or newly diagnosed with hypertension consulting at the General medicine department were enrolled based on inclusion and exclusion criteria. RESULTS: In the study 100 patients were enrolled, of which 43 were males and 53 were females. Most patient were found to be in pre- hypertensive and stage- 1 of hypertension both being 33%. The most frequently prescribed drugs as monotherapy were telmisartan (27%), amlodipine (23%) and atenolol (6%). The use of ARB was prominent in monotherapy, dual therapy and combination therapy. 78% of the studied prescription were in adherence to the JNC-8 guidelines for prescribing antihypertensives. 73% of the hypertensive population were eligible for estimation of FRS. CONCLUSION: The prescribing pattern at the research site adheres to the JNC-8 prescribing guidelines for hypertension with monotherapy, dual therapy, combination therapy being prescribed in accordance with hypertensive stage of patients. A major proportion of study population was found to be more than 40 years of age, indicating age as a strong factor in prevalence of hypertension. Female population was found to be dominant in comparison with male population with the ratio of 1.12:1. Assessment of individual 10-year risk for developing CVD concluded that majority of the hypertensive population was prone. Both age and gender play a vital role in development of cardiovascular complications.
