Medication Therapy Management for the Ageing Population: A Pharmaceutical Care Plan

Abstract: Older patients undergo a change in their drug metabolism and pharmacokinetic variation with the increasing age. Complex medication regimen due to their multiple chronic conditions is needed to manage improved outcomes. The development of poor medication adherence due to side effects, forgetfulness, and drug-related problems make it difficult in the management of medications and pose a great challenge across the globe, having a significant impact on public health. Unawareness on the part of health professionals and low health literacy in geriatrics also contributes to the growing concern. Medication therapy management (MTM) is an elaborate extension of the practice widely known in the 1990s as pharmaceutical care which was officially recognized by the United States government in 2003. This program focuses on delivering a diverse non-dispensing clinical service to patients and the clinicians. These services include education on disease state, medication assessment, identification of drug-related issues, and improving patient adherence. MTM is a five-step process including medication therapy review (MTR), personal medication record (PMR), medication-related action plan (MAP), intervention, documentation, and follow-up. It also facilitates the optimization of medication used to reduce the risk of severe ADRs in elderly patients. Home medication review conducted by pharmacists has reduced drug therapy problems, improved patient compliance, and medication adherence. In developing countries like India, the introduction of MTM can effectively manage healthcare complications by providing geriatrics with a better quality of life and reduced financial burden for the treatment. Interdisciplinary efforts by physicians, pharmacists, nurses, and other healthcare providers can serve as an effective intervention in reducing the disease burden and drug-related complications for a better health care system in India.
