Time to Success and Its Prognostic Factors among Patients with Myringoplasty Surgery

Abstract:

Myringoplasty is a surgical ear procedure indicatedto treat perforation of tympanic membraneandto prevent
further ear infection and improving hearing. Success rate has been a concern for patients with different
indications. This study aims to determine the time to success in myringoplasty surgery and to
identifyprognostic factors contributing to its success. A retrospective cohort study was conducted on 215
patients diagnosedas tympanic membrane perforations and with myringoplasty surgery done from 2008 to
2017. Results from Kaplan Meier analysis showed overall median time to success in myringoplasty was 19.9
(95%CI: 13.6, 44.0) months. Multiple Cox Proportional Hazard Regression revealed that age, gender, smoking
status, upper respiratory tract infection (URTI), size of perforation, affected side, graft used, and rank of
surgeon were significantly associated with success rate of myringoplasty. Success of a myringoplasty appeared
to be lower than commonly quoted success rates due to difference of patient’s selection criteria, the definition
of success as well as the duration of follow up after surgery. Time to success, postoperative expected outcome
and the prognostic factors should be properly counselledto patients
whentakingintoaccountofvariousdefinitionsofsuccessasunrealisticsuccess expectation is contraindicated for
myringoplasty.
