Three-dimensional ultrasonography versus office hysteroscopy for the evaluation of uterine cavity and cervical canal before IVF

Abstract:

Objectives: to compare between 3D TVS & office hysteroscopy to evaluated uterine cavity and cervical canal
in females with normal 2D TVS & HSG scheduled for IVF.
Methods: Observational cross-sectional investigation was carried out in Cairo University Obstetrics and
Gynecology Hospital. We included 310 patients attending to infertility clinic pre-IVF, with normal 2D TVS &
HSG. Office hysteroscopy & 3D TVS were done on the same day and results are compared.
Results : 3D TVS has high sensitivity (87.1 %) , specificity ( 100% ) , positive predictive value ( 100%) , negative
predictive value ( 94.76%) with overall accuracy 96.13% .The most common hysteroscopic finding was
endometritis 17 cases (5.4%) , while uterine polypi and endometritis were the most common 3Dultrasonographic pathologies 13 cases (4.2%) each .Significantly differs between OH and 3D TVS was observed
regarding previous ART trials ( P values 0.041 and 0.920 for patients with and without history of previous ART
, respectively. The most common findings in patients with previous IVF trials (87 patients) were endometritis,
uterine polypi and intrauterine adhesions, 4 cases (4.6%) each.
Conclusion: 3D TVS can be used as a preliminary diagnostic tool or complementary to office hysteroscopy
for cervical canal evaluations and uterine cavity in patients with normal HSG & 2D TVS, scheduled for IVF. The
use of office hysteroscopy may be reserved for patients with previous ART failure.
