Evaluation of the effectiveness of surgical treatment of vitiligo by melanocytic transplantation based on the study of TRP-2 molecule expression

Abstract:

Introduction. This article presents the results of melanocytic transplantation in vitiligo, taking into account the expression of one of the key proteins of melanogenesis - the tyrosinase enzyme associated with the TRP-2 protein. Activation of TRP-2 molecules and their detection as the main biomarker of melanocyte stem cells and an indicator of proliferative activity in the area of the hair follicle bulb is the main pathogenetic mechanism of the development of repigmentation after autotransplantation. The aim of this study was to assess the clinical efficacy of autologous uncultured melanocyte transplantation methods based on the study of the expression level of protein-bound tyrosinase (TRP2). Material and methods. The biopsy of the affected skin area was performed in 70 patients diagnosed with vitiligo, followed by an immunohistochemical analysis. In order to compare the expression of the TRP-2 molecule, the patients were divided into 4 groups. Results. Surgical treatment of vitiligo with uncultivated keratinocyto-melanocytic suspension (NCECS) and together with hair follicles (NCECS + NCORSHFS) was carried out by transplantation of autologous uncultivated epidermal cell suspension (NCECS) and in combination with dermal cell suspension of hair follicles (NCORSHECS + NCORSHFS). Cells and other precursor elements were concentrated by a mechanical process, which made it possible to obtain a cell suspension that can be used for tissue regeneration, in this case, a suspension of healthy hair follicle grafts containing mesenchymal cells of melanocyte precursors. Conclusion. The results of the high efficiency of uncultivated automelanocytes together with the dermal cell suspension of hair follicles (NCECS + NCORSHFS) proved not only the significance of the TRP2 protein in enhanced differentiation of melanocyte cells, but also a high degree of repigmentation in vitiligo foci, which opens up further prospects for pharmacological regulation of this protein in vitiligo therapy.
