Based on Riskesdas in 2018, about 32.3% of hypertension sufferers did not comply with the medication. About 13.3% of people diagnosed with hypertension also did not take any medication. This study aims to determine the adherence medication determinants of hypertensive patients in urban and rural areas of South Sulawesi Province. This study used an observational analytic with a cross-sectional study design. This study population was 5936 people with hypertension in urban and rural areas. The number of samples was 365 hypertension patients, obtained using proportional stratified random sampling. The data analyzed by bivariate Chi-Square analysis and multivariate logistic regression. The study showed the determinants associated with medication adherence to hypertension sufferers were including, an education level (p= 0.000), access to health services (p= 0.000), family support (p= 0.000), the most influential determinants on treatment compliance in urban areas and rural was education (OR= 11.780; 95% Cl = 6.615-20.976). The study revealed the level of education, access to health services, and family support related to hypertension sufferers' compliance in urban and rural areas. Gender and health insurance were not correlated with compliance with hypertension medication in urban and rural areas. It is recommended that each public health service conduct strict medication adherence to patients diagnosed with hypertension, particularly for low-level education patients. Also, a drug-taking supervisor should be established.
