Abstract:

Chrono therapeutics is a unique approach in the delivery of therapeutic drugs wherein release of drug occur as per the symptoms of disease to improve effectiveness and minimize side effects. Time at which drug has been administered in the body have significant effect on its efficacy. If drug released at predetermined rates it ultimately exposes the patient to threshold concentration when required. Appearance of symptoms of a disease is affected by biological rhythm of body or circadian rhythm which is controlled by suprachiasmatic nuclei in hypothalamus and pineal gland. So now a days many treatments are based on circadian cycle of disease and improved patients’ outcomes have been demonstrated in clinical trials, mainly for cancer, respiratory and hypertensive diseases. Biological rhythm is classified as ultradian, infradian, circadian on the number of hours and circaseptan, circamensual, circannual based on days, weeks, and years respectively. Chrono kinetics deals with the alteration in absorption, distribution, metabolism, and elimination because of dosing time. Chrono dynamics accounts for dose time dependent change in drug-receptor interaction, free to bound drug, protein binding, volume of distribution, etc. Basic requirement to design a chrono therapeutic drug delivery system is knowledge of circadian time structure and biological rhythm, disease pathophysiology or variation in symptom around 24 hour and chrono pharmacology of drug to be given. Approaches designed as chronotherapeutic drug delivery system with drug, polymer, type of delivery system, release kinetics, and stability has been delineating in this review.
