The immune system plays an important role in protecting the body from various infectious diseases such as tuberculosis, which is still quite prevalent in Indonesia, so optimizing the immune system's function is very important by giving immunomodulators. The chemotaxonomic approach allows the fruit of Etlingera rubroloba A.D. Poulsen to contain secondary metabolites as immunomodulatory agents. This study aims to determine the immunomodulatory potential of E.rubroloba A.D Poulsen fruit fraction against macrophage phagocytosis activity and levels of Interleukin-12 (IL-12) in balb/c mice. The 45 test animals were divided into nine treatment groups, namely normal control, negative control (Na-CMC 0.5%), positive control (stimuno®), Fractions A, B, C, D, E, and F (200 mg/kgbw). The treatments were given daily for seven days orally, and on the eighth day, all groups were injected with BCG vaccine 0.1 mL intraperitoneal except for the normal group. Macrophage phagocytosis activity was calculated from peritoneal fluid smear, and IL-12 levels were measured by the ELISA method. Data were analyzed by one-way ANOVA and continued with the post hoc tukey test. The results showed that the highest percentage of macrophage phagocytosis activity and IL-12 levels were infractions C, A, and D. Post hoc Tukey statistical test showed significant differences between fraction groups with positive control (P<0.05) on immunomodulatory potency based on increased macrophage phagocytosis activity and IL-12 levels in Balb/c mice.
