Effect of oral isotretinoin on retinal nerve fibre layer and ganglion cell layer thickness

Abstract:

Background: Isotretinoin use in severe recalcitrant acne is likely to be associated with lot of anterior segment ocular problems like dry eye, blepharitis, keratitis, corneal opacities etc. However, its effect on inner retina has not been studied in detail.
Objective: To study the effect of oral isotretinoin on the peripapillary retinal nerve fibre layer and macular ganglion cell layer thickness.
Methods: A prospective longitudinal study conducted at a tertiary eye care centre in Eastern India to study the effect of 0.5mg/kg/d oral isotretinoin on the retinal tissue using spectral domain optical coherence tomography. Peripapillary retinal nerve fibre layer and macular ganglion cell layer thickness was measured at baseline and 4 months after isotretinoin therapy. The data obtained was compared using student t test and chi square test and a p value of <0.05 was considered significant.
Results: 33 eyes of 33 patients (M= 16, F=17) with average age of 21.27 years were examined using spectral domain optical coherence tomography. No significant differences were found between pre and post treatment measurements across any of the variables.
Conclusion: Oral isotretinoin at a dose of 0.5mg/kg/d over a period of 4 months does not affect inner retina.
