The relationship between NUCB2/Nesfatin-1 in the Follicular Fluid and the Blood in Iraqi women with Polycystic Ovary Syndrome compared to Poor Ovarian Response undergoing Intracytoplasmic Sperm Injection

Abstract: Background: Polycystic ovary syndrome (PCOS) is a significant endocrinological condition in reproductive-age females which is typified by ovulatory dysfunction, hyperandrogenism, and polycystic ovarian morphology (PCOM). POR is accountable for nearly 5-18 percent in all ART cycles associated with a low pregnancy rate of 2 – 4 percent .Poor ovarian responders (POR) is a critical success-limiting factor for ICSI/IVF results. POR has been described with varying definitions. The standardized definition of POR as BOLOGNA criteria was published by ESHRE in 2011. Recently, nesfatin-l has been implicated in the maintenance of pregnancy due to the increase in the NUCB2/nesfatin-l levels. Methods: This case-control prospective research including 88 admitted patients at Kamal Al-Samaraee Hospital For Infertility Management And IVF And The Higher Institute For The Diagnosis Of Infertility And Techniques Of Assisted Reproduction in Baghdad Iraq for the period between October 2019 -April 2020, forty four patients were diagnosed with poor ovarian responders based on Bologna criteria and 44 patients diagnosed with PCOS according to Rotterdam criteria. All undergoing ICSI by controlled ovarian stimulation and flexible antagonist protocol aspiration of follicular fluid (FF) and oocytes retrieval obtained from mature sized follicles (=17 mm diameter). blood sample also is taken at second day of the menstrual cycle for measurement of E2 , progesterone ,FSH ,LH ,AMH, fasting glucose and testosterone another blood sample is taken at day of trigger for measurement of estradiol .blood sample is taken at the day of ova pick up and stored for measurement of nucleobindidn/nesfatin analyzed by using ELISA. follow up of all patients is done for two weeks after fresh embryo transfer and two weeks later for TVU to confirm pregnancy Statistical analysis was performed with SPSS version 20. Results: A positive correlation between blood NUCB2/Nesfatin-1 and E2 (r=0.07, p=0.62)), FSH (r= 0.11, p=0.47), fasting glucose (r=0.01, p=0.9)), BMI (r=0.16, p=0.28)) in PCOS was found, but it was not significant. Blood NUCB2/ Nesfatin-1 concentration were lower in PCOS subjects than the POR group, and were statistically significant (p=0.008). NUCB2/ Nesfatin-1 levels FF were seen to be higher in PCOS subjects than the POR group, and were statically significant (p=0.033). Blood NUCB2/Nesfatin-1 levels were significantly higher than FF NUCB2/Nesfatin-1 levels in both groups .
