Estimation of the diagnostic value of amino acid composition of oral fluid and blood serum in children with dental erosion and their effectiveness of pathogenetic treatment

Abstract: In recent decades, the problem of dental erosion has attracted great professional interest among clinicians and scientific researchers. This is due to the fact that the general prevalence of diseases of hard tissues of teeth of non-bacterial origin, among which erosion, hypoplasia, wedge-shaped defect are more often diagnosed, is steadily growing. The aim of the study. To assess the prognostic significance of the use of pathogenetic therapy for erosion of hard dental tissues. Materials and research methods. The object of the study was 240 children aged 2 to 6 years, the average age was 4.4 ± 0.4 years, who applied to the department of the clinic of pediatric therapeutic dentistry of the Tashkent State Dental Institute and the department of the clinic of the Bukhara Regional Children's Dental Clinic for the period from 2016. ... to 2020. All children were divided by a randomized method: group 1 - 50 children without dental pathology; Group 2 - 125 children with erosions of dental hard tissues; Group 3 - 65 children with combined lesions of teeth with erosion and multiple caries. During the study, indicators of thyroid status, levels of calcium regulating hormones, and biochemical markers of bone metabolism were studied. Determination of the content of vitamin D, PTH and osteocalcin in saliva was carried out by the method of enzyme immunoassay on a Mindray analyzer using ready-made reagent kits 25-Hydroxy Vitamin DEIA, DRGPTH Intact (eIa-3645) and N -O MIDocalcin ELISA. Determination of calcium and phosphorus was carried out using ready-made kits (Human). Research results. In the study, the level of individual amino acids present in a free state in the enamel in children with erosion of hard tooth tissues is 1.5-2 times higher. It is likely that these are free amino acids and amino acids included in proteins not associated with minerals. Under the influence of various factors, their denaturation is possible, a way opens up for action on hydroxyapatites, and this leads to fragility of the teeth and a tendency to crack formation. It is known that the amino acid lysine is involved in the formation of salt-like bonds with inorganic components, forming primary foci of mineralization, and serine is involved in calcification processes. An increase in the content of lysine, oxylysine and serine promotes increased mineralization and stronger adhesion of the mineral phase to the protein matrix. It should be noted that a comparative study of the protein mass fraction and the amino acid composition of the oral fluid and blood serum revealed statistically significant differences in children with erosion of the hard tissues of the tooth enamel. At the same time, in our opinion, a genetically determined suppression of protein synthesis and an imbalance of the amino acid composition, especially of such amino acids as histidine, glycine, glutamic and aspartic acids, alanine, phenylalanine, leucine, lysine and cysteine, entail disturbances in the processes of protein synthesis and mineralization and as a consequence, the implementation of the erosion of the hard tissues of the teeth. Conclusions: Thus, the progression of metabolic disorders in children with erosion of hard tooth tissues indicates an increase in osteoporosis and demineralization, a slowdown in synthetic processes in hard tissues of teeth and bone tissue. The established pathogenetic complex has a negative conjugate effect on enamel resistance, which accordingly requires correction in this contingent of children.
