Abstract: The material for the analysis was the results of examination and treatment of 220 patients with fractures of the bones of the lower extremities, who were in the clinic of the Republican Specialized Scientific and Practical Center of Traumatology and Orthopedics from 2004 to 2020. open fractures were observed in 67 (30.5%) victims. The severity of open fractures was assessed according to bay A.V. Kaplan, O. N. Markova (1975). the most common injuries in patients were type I (26 - 38.8%) and II (23 - 34.3%). Damage of type III (18 - 26.9%) was registered somewhat less frequently. fat embolism developed in 28 (12.7%). Out of 147 patients with fractures of the bones of the lower extremities, 102 patients used external fixation devices based on wires and rods. A wire rod apparatus (Patent: FAP 00738, 23.06.2012) and an algorithm for diagnostics and treatment of open fractures were developed. (DGU Patent: 0918122.09.20) Treatment results were assessed according to the criteria of E.R. Matthies (1985). The use of the developed combination of external and internal fixation devices, based on their choice according to the indications, made it possible to obtain positive results up to 83.3%.
