Improvement of Anterior Extraperitoneal Approaches in The Surgical Treatment of Tuberculosis of The Lumbar and Lumbosacral Spine

Abstract:

The study is based on the data of examination of 168 patients with tuberculosis spondylitis (TS) of the lumbar and lumbosacral spine, who underwent surgical intervention using anterior pararectal - in 141 (83.9%) and 27 (16.1%) cases with anterior transverse access to the anterior abdominal wall. In 101 (60.1%) patients, radical reconstructive surgery (RRT) was performed in the lumbar spine, and in 67 (39.9%) cases - in the lumbosacral spine. Of these, 161 (95.8%) patients with the affected segment underwent fusion using a titanium mesh cage (Piramesh), and in 7 (4.2%) patients - the traditional classical method with autologous bone fusion. The use of anterior extraperitoneal approaches allows the surgeon to work fully in the lumbar and lumbosacral spine, but the risk of damage to the great vessels, since the abdominal aorta, inferior vena cava, iliac arteries and veins, and the peritoneum is high and requires a deep knowledge and experience of the operating surgeon. Using anterior extraperitoneal approaches, damage to the muscles of the anterior abdominal wall, nerves and blood vessels, muscle prolapse, hernia of the abdominal wall, discomfort, and a rough scar are not observed.
