Abstract: Introduction: The laboratory diagnosis evidence of enteric fever dependent heavily on serological test i.e. the Widal test due to lack of facilities of culture in rural healthcare facility of India. Objectives: To determine the baseline Widal titre of antibodies to the O-antigen, H-antigens of S.typhi and H- antigens of S. paratyphi A and B amongst apparently healthy individuals residing in rural area of Western Maharashtra state. Methods: A cross sectional study was conducted in rural tertiary healthcare center located Western Maharashtra. Non –replicative blood specimen was collected from apparently healthy blood donors of blood bank during April 2013 to October 2014. A total of 228. Blood samples of healthy donors were tested by Widal test. Results: Out of 228 samples 32% (73/228) were positive for O antigen of S. typhi. The frequently recorded “O “antibody titre of positive samples was found to be 1:20(78%, 57/73) and this was considered as baseline titer for O agglutinins. Out of 228 healthy donors 26.7% (61/228) were positive for agglutinins against H antigen of Salmonella typhi. The most frequently shown titre for H was 1:20 (86.8%.53/61), followed by 1:40 (6.5% , 4/61) in healthy donors. No agglutination was found to Paratyphi AH and Paratyphi BH antigen. Conclusion: Study observations reviled baseline antibody titre for salmonella enterica subsp in healthy donors was 1:20 for O and H in this geographical area.
