Prognostic importance of hyponatremia in acute st- elevation myocardial infarction

Abstract:

The purpose of the assessment was to choose the prognostic criticalness of hyponatremia in exceptional ST elevation myocardial dead tissue for foreseeing early mortality. We studied 100 consecutive patients with acute ST elevation myocardial infarction in Coronary Care Unit of Tertiary care hospital. The study population were divided into three groups depending upon serum sodium levels. The data of the study was analyzed with respect to various risk factors in association with development of hyponatremia.There was no correlation of age and sex in development of hyponatremia.Out of diabetic patients, 42.10% patients developed hyponatremia and 28.57% tobacco users developed hyponatremia which was not significant. Amongst 23 hypertensive patients, 5 patients were on antihypertensive dose of diuretics. There is no significant statistical difference. (P value 0.4813)
The mortality in patients with hyponatremia on admission was 27.30% which was significantly higher (P value 0.0073) than in patients who develop hyponatremia within 72 hrs. Of admission. (17%) Patients with more severe hyponatremia had higher mortality. (P= 0.0032).CONCLUSION the study concluded that, hyponatremia on admission or early development of hyponatremia in patients with acute ST segment elevation myocardial infarctions correlates with severity of LV dysfunction (in term of LVEF) and can be considered as an independent marker of early mortality and the expectation decays with reality of hyponatremia.
