A Comparative Study on Efficacy of Topical Ketorolac Tromethamine 0.4% W/V And Flurbiprofen 0.03 % W/V In Maintenance of Mydriasis During Uneventful Cataract Surgery

Abstract: Maintenance of mydriasis during intra-operative cataract surgery is very essential. The topical NSAID’S like ketorolac tromethamine and flurbiprofen are potent cyclo-oxygenase enzyme inhibitors, thus they are inhibiting prostaglandin’s biosynthesis. Constriction of the pupil during phacoemulsification and small incision cataract surgery (SICS) is found to be the major cause of iris damage, iris chafing, incomplete cortex removal, posterior capsular rupture, vitreous loss. So the dilation of the pupil is necessary. The present study was conducted to compare the efficacy of topical ketorolac tromethamine 0.4 % w/v and flurbiprofen 0.03 % w/v in the maintenance of mydriasis during phacoemulsification and SICS. A prospective observational study was performed on 160 patients for a period of 6 months from November 2018 to April 2019. Based on inclusion and exclusion criteria, 80 patients were allocated in two groups and were given either topical ketorolac tromethamine or flurbiprofen before cataract surgery. After instilling the drugs the pupil diameter was measured during pre-operative, intra-operative, and post-operative stages of surgery. The pupil diameter during pre-operative surgery was 7.54±0.698 mm for flurbiprofen and 7.628±0.858 mm for ketorolac, intra-operative surgery was 7.98±0.599 mm for flurbiprofen and 8.256±0.636 mm for ketorolac, and at the end of surgery was 6.42±0.737 mm for flurbiprofen and 7.15±0.98 mm for ketorolac. Hence the above study may be concluded that ketorolac tromethamine 0.4% w/v was found to be effective than flurbiprofen 0.03 % w/v in maintaining mydriasis during cataract surgery.
