Assessment of cardiovascular involvement in hypothyroidism

Abstract:

Background and Aim: Hypothyroidism, ie, reduced thyroid hormone secretion, decreases the metabolic functioning, which further lowers the cardiac and other multiorgan involvement. Timely diagnosis and treatment of hypothyroidism can prevent additional complications imposed on the cardiovascular system. Therefore, the objective of this study was to assess the cardiovascular manifestations in hypothyroid patients.
Methodology: Hundred subjects presenting with early hypothyroidism were selected, and their demographic (gender and age) and clinical data were recorded. Serum thyroid function test (T4, T4, and TSH levels) and electrocardiographic studies for the cardiac output were measured. 2D transthoracic echocardiography and chest radiography were also performed to study the cardiac involvement. R software was used to analyze the data and independent t-test and Mann-Whitney U test were employed to compare the data.
Results: The mean age of the patients was 39.59 ± 13.96 years, with female subjects being predominant. The gender ratio of subjects with hypothyroidism was 1:4.5 (male: female). Out of 100, only 49 subjects showed moderate levels of TSH (20-50 micIU/mL) while it was severe in 16% of the subjects (>50 micIU/mL). Abnormal cardiac functions like pericardial effusion, diastolic dysfunction (Grade I), and increased interventricular septal thickness (>1 cm) were observed in hypothyroid patients with TSH >50 micIU/mL.
Conclusion: Early diagnosis and treatment of hypothyroidism is needed to prevent cardiovascular complications.
