Frank’s sign or diagonal earlobe crease sign (DELC) indicates the presence of a diagonal earlobe crease which is associated with coronary artery disease (CAD). But however due to lack of proper theoretical background for the association between DELC and atherosclerosis remains controversial. Hence the aim of the study is to evaluate the association between DELC and CAD in dental patients. A case control study was conducted on patients who visited the Department of Oral Medicine, Radiology and Special Care dentistry during the period of June 2019 to March 2020. 50 patients who had undergone angiography and diagnosed with coronary heart disease(CHD) were included in the case group. 50 patients without CHD and had no history of hypertension, diabetes and stroke were included in the controls group. All 100 patients were examined for the presence of diagonal earlobe crease. Descriptive statistics and chi-square analysis were used to interpret the results. 70% of the cases and only 20% of the controls had Frank’s sign. The prevalence of DELC was seen higher in patients diagnosed with CHD(cases) than when compared to the controls. Frank’s sign may be a useful indicator of CHD in patients visiting dental clinics.
