Comparative Evaluation of The Accuracy of Working Length Determination by Radiographic Method and Electronic Apex Locator

Abstract: The apical termination of the biomechanical preparation and obturation is the most important factor influencing the success of root canal treatment. Determining the correct working length is the key element in achieving this crucial factor. Various methods have been used to identify the correct measurement. Conventional radiographs are by far the most common and simple method but sometimes image superimposition and anatomic interferences can be problematic. Recently electronic apex locators are widely preferred in determining working length. The aim of our study was to evaluate and compare the accuracy of working length determination by radiographic method and electronic apex locator. A total of 25 single rooted vital teeth scheduled for extraction was chosen. Standard access cavity preparation was done for each tooth and a straight line access to the root canals was achieved. The working length was determined by both the methods after which the teeth were immediately extracted. The actual working length was determined with a file under magnifying loupes(3X). The result data was tabulated in excel and imported to SPSS for statistical analysis. ANOVA test was done and P<0.05 was considered to be the level of statistical significance in our study. It was observed that the electronic apex locator had 98% significant acceptable range from the radiographic method(96%) when compared to the actual measurement on the extracted tooth. The electronic apex locator method demonstrated significant accuracy in determining the working length and it can be used as a good adjunct to radiograph to prevent failure of the endodontic treatment.
