Clinical practice guidelines in the management of open apex cases

Abstract:

Open apex cases are frequently seen in the younger age group patients , where the closure of the root apex has been hampered due to a history of trauma. In such cases the root walls of the radicular dentine remain parallel and do not close forming a proper apical closure.
The trauma induced in these teeth leads to the formation of the necrotic tissue which further leads to progression of inflammation through the prostaglandin and interleukin based inflammatory pathways.The management of such cases at the earliest is critical so that the bone resorption associated with these inflammatory signals is managed.
In management of open apex cases the operator has to be very careful while performing the procedure of canal debridement as there is an increased risk of apical extrusion of these irrigating solutions and medicaments.The extrusion of these irrigating solutions and medicaments leads to further tissue necrosis of the associated adjacent periodontal tissues , which causes further poor prognosis out of the treatment rendered. Often the patient reports with pain and fever in such cases. Therefore it is relevant to formulate the correct protocol in the management of the open apex cases so that after the canal debridement regeneration of the periodontal tissues occurs which would restore the periodontal apparatus to its optimum health. So the aim of writing this paper is to discuss and formulate the standard clinical guidelines and various strategies for management of the open apex cases so that the favourable results can be achieved in a shorter time period by forming the apical barrier which is free of infection.
