Traumatic injuries to the primary dentition presents with a special challenge to the dentist and creating panic to the parents. Root fracture in primary teeth is a rare occurrence due to the plasticity of the developing alveolar bone. Tooth with root fracture are often untreated due to anticipated risk of injuring underlying permanent tooth. There is no report on the management of apical root fracture in primary incisor with an evidence based follow up to assess the fate of fractured apical fragment. This paper reports a case of primary central incisor with apical root fracture with three years follow up. Documentation of modified treatment protocol with favorable prognosis will help the clinicians to preserve primary teeth until its normal exfoliation.
