Changes in the Parameters of Gingival Crevicular Fluid in Masticatory Function Restoration by Various Prosthodontic Constructions

Abstract:

Objectives
Widespread use of fixed prosthesis in prosthodontic dentistry requires considerable preparation of the hard tissues of the tooth with crown margin insertion into the gingival sulcus which often causes marginal periodontitis. The objective of the research is to study the dynamic peculiarities of gingival crevicular fluid parameters in restoration by fixed prosthodontic constructions of various manufacturing techniques.
Methods
The gingival crevicular fluid was studied in 105 patients, divided into three groups, before the treatment as well as 6 and 12 months after the masticatory function restoration by various prosthodontic constructions. The gingival crevicular fluid was studied by determining the pH, impregnation surface of the filter paper and volume of gingival crevicular fluid. In total, 315 portions of gingival crevicular fluid were studied.
Results
The study of gingival crevicular fluid pH revealed that after the restoration of masticatory function with artificial crowns of various frames and manufacturing technologies, this index increases in comparison with the initial value in almost the same proportions – up to 1.1 folds, compared with the data obtained before the restoration both in the first six months after fixation and a year later (?<0.001).
Significance
Fixed prostheses (all-metal, metal-ceramic, metal-acrylic, all-ceramic) are widely used in orthopedic dentistry. A study of the dynamics of the parameters of the amount of gingival crevicular fluid before and after the restoration of chewing function showed that it is best to use structures based on zirconium and cobalt-chromium made using computer-aided design and manufacturing (CAD/CAM) technology.
