Mechanism of Multi Drug Resistance and Chemotherapy -A Review

Abstract: Multidrug resistance (MDR) to chemotherapy remains a major challenge in cancer treatment. Resistance to any effective anticancer medication may be expanded by various pathways including decreased drug intake, multiple drug efflux, activation of detoxifying processes, activation of DNA restore pathways, avoidance of drug-borne apoptosis, etc. In the first part of this chapter, we briefly summarize current information on individual cellular mechanisms responsible for MDR, with special emphasis on ATP-binding cassette transporters, perhaps this textbook's main theme. Although substantial work has been done to characterize MDR mechanisms in vitro, translation of this knowledge to clinic has not been successfully completed. Therefore, the discovery of genes and pathways critical to the production of MDR in vivo and the establishment of a robust method for the analysis of clinical samples may help to predict resistance progress and lead to therapies to circumvent it.
