HYPOTHYROIDISM AND HEART: A Cross Sectional Study of Cardiac Manifestations.

Abstract: Hypothyroidism is a vastly prevalent endocrine disorder with wide array of symptoms and signs with multi system involvement. One of the most critical system involved is cardiovascular system. So, my aim of this study is to estimate the prevalence of cardiovascular involvement in hypothyroid patients. This cross-sectional study was carried out in 104 patients with hypothyroidism in the Department of General Medicine, SBKS MIRC, Sumandeep Vidyapeeth over a span of one and half year. It included detailed profile of hypothyroid patients and cardiovascular evaluation was done through clinical examination, ECG and 2D echocardiography. Statistical data analysis was done using Frequency Distribution, Mean, Standard Deviation. Among 104 patients, mean age of the patients was 38.11±11.70 years with Female predominance in 76.9% of the total population. Three fourth of the study population belonged to rural regions. Most common presenting clinical features were generalized weakness and tiredness (76.9%),dry skin (59.6%). Classic finding of CNS examination hung up reflex was seen in 44% of the patients. Diabetes mellitus type 2(20.1%) and Hypertension (11.5%) were the major comorbidities seen. 72.2% of the patients had hypothyroid dyslipidemia.51.93% of the patients had ECG abnormalities in the form of bradycardia (24.03%)followed by low voltage complexes, Nonspecific ST-T changes, RBBB and LBBB. But only 21.16% of the patients had 2D echo abnormalities. Most common aberration was Diastolic Dysfunction (15.38%)in study population followed by pericardial effusion and systolic dysfunction. Hypothyroid patients present clinically with various nonspecific signs and symptoms which are difficult to attribute to cardiovascular involvement. Hence, a high index of suspicion is the key for early diagnosis of cardiovascular involvement in hypothyroidism.
