Features of perioperative management of children with diffuse appendicular peritonitis

Abstract:

402 children with widespread appendicular peritonitis (WAP) were examined and treated. Short-term preoperative preparation was performed in 138 patients with RAP of the comparison group. Perioperative measures were carried out in 264 patients of the main group in the intensive care unit under constant clinical and laboratory monitoring, taking into account the pronounced changes in the body of a sick child: the state of acid-base and electrolyte balance, hemostasis, as well as tissue perfusion. Patients of the main group also underwent adequate complex intra- and postoperative intensive therapy using ozonized crystalloid solutions. The developed scheme of perioperative management of children with WAP allowed to reduce the average indicators of the duration of the operation, the length of stay in the intensive care unit, the duration of postoperative analgesia, the terms of normalizing body temperature, contributed to the earlier restoration of motor-evacuation function of the intestine, the activation of patients, the reduction of the terms of inpatient treatment and mortality of patients.
