Comparative evaluation of the effectiveness of Cyclosporine A and Tacrolimus in the treatment of idiopathic nephrotic syndrome in children

Abstract:

Short-and long-term efficacy and safety of tacrolimus (1st group - 30 patients) and cyclosporine A (2nd group - 24 patients) in the induction and maintenance of remission in 54 children with idiopathic nephrotic syndrome occurring in steroid-resistant (SRNS) or steroid dependent (SDNS) forms. In children with SRNS, we found that tacrolimus had better short-term effectiveness then cyclosporine A. However, when evaluating the long-term effectiveness in both groups of children with SRNS, we did not determine differences in the frequency of relapses. There was no significant difference in long-term effectiveness in the group of children with SDNS, determined by the frequency of relapses in children with complete remission in the observed two groups. Moreover, it was found that there were no significant differences in short-term remission or relapse rates in both groups of children with SDNS. However, tacrolimus in the treatment of SRNS showed higher efficacy with a low degree of nephrotoxicity compared to Cyclosporine A. Despite the presented advantage over Cyclosporine A, we concluded that Tacrolimus is not always the best choice in the treatment of SDNS.
