The peculiarities of endothelial dysfunction indicators in patients with chronic brain ischemia

Abstract: To accomplish this goal, 28 patients with chronic brain ischemia were under our care. Patients were on outpatient treatment in the SSMI clinic. Patients with CBI at the age of 40-65 were mostly suffering from cerebral circulation disorders due to atherosclerosis of cerebral vessels and were undergoing outpatient treatment. Concomitant arterial hypertension was observed in 78.8% of the patients. The criteria for exclusion were the age over 70 years, myocardial infarction in the history of at least 6 months, insulin-dependent diabetes, the presence of malignant tumours. In this contingent of patients with CBI, ultrasound Dopplerography of cerebral vessels was used to determine the thickness of the CBI. The control group consisted of 12 healthy individuals. All examined patients underwent a dental and biochemical study. Dentist examinations were performed using a standard technique in a dental chair with artificial lighting and using a standard dental kit for the examination of patients. To evaluate the condition of the oral mucous membrane, the following cracks of the red border of the lips, candidiasis glossitis, angular heilitiswere registered in the examined patients. For assessment of the state of vascular endothelium in patients with CBI the blood was taken in the morning hours, on an empty stomach, gravity into a plastic tube. Blood for the study was taken from the elbow vein twice: in the amount of 10 ml to the cuff sample (3-5 minutes of pressure on the shoulder vessels with the sphygmotonometer cuff) and 5 ml after the cuff sample. Analysis of the obtained biochemical results showed that changes in the immune properties of the endothelium in CBIare accompanied by its damage and dysfunction. Thus, as a result of studies, it was found that patients with CBI have a decrease in thromboresistance of the vascular wall, which is manifested by inhibiting the anticoagulant and fibrinolytic activity of the endothelium. The study showed that the inflammatory process in OMM in CBI contributes to the development of oxidative stress in the vascular wall. Therefore, the induction of oxidative stress in OMM epithelial cells may be a potential mechanism for the detected increase in endothelium I concentration during CBI. Thus, the results of our research allow us to conclude that patients with CBI have an increase in the content of markers of endothelial dysfunction, which is associated with a direct dependence of clinical manifestations of chronic brain isCBIia in the oral cavity.
